Nature of the work

As a Community Support Services Worker, you will provide direct bilingual services to eligible clients, including seniors and adults living with physical disabilities. You will conduct assessments, coordinate resources, and collaborate with community partners to meet client needs — all while ensuring that services are delivered with empathy, dignity, and respect.

Responsibilities

  • Meet with clients in person or by phone, involving family members or caregivers when appropriate.
  • Conduct client intakes and needs assessments through one-on-one interviews.
  • Maintain ongoing communication and follow-up with clients to ensure continuity of care.
  • Provide information, support, and advocacy to help clients access community, health, legal, and financial services.
  • Mobilize and coordinate appropriate resources to meet client needs.
  • Make and receive referrals to ensure seamless integration of services.
  • Handle client complaints, cancellations, and concerns with professionalism and tact.
  • Respond promptly to emergencies identified by staff, clients, volunteers, or caregivers.
  • Complete and submit all required client documentation and forms.
  • Maintain accurate and up-to-date client files, ensuring strict confidentiality.
  • Prepare and maintain necessary reports, records, and statistics.
  • Keep the Community Support Services Coordinator informed of program activities, issues, and emerging trends.
  • Coordinate volunteer involvement in client support, including supervision and quality assurance.
  • Participate in the planning, development, and delivery of program services.
  • Build and maintain strong relationships with community partners and stakeholders.
  • Actively participate in team meetings, professional development activities, and centrewide initiatives.

What we would like to find in you

  • Bachelor of Social Work (B.S.W.) degree or equivalent college diploma with relevant experience.
  • Minimum two years of experience delivering community support services or programs for seniors or adults with physical disabilities.
  • Strong understanding of the unique needs of seniors and adults with physical disabilities.
  • Demonstrated ability to work effectively as part of a multi-service, collaborative team.
  • Ability to work independently, show initiative, and manage competing priorities.
  • Commitment to providing high-quality, client-centred services.
  • Excellent organizational, communication, and problem-solving skills.
  • Fluency in oral and written communication skills in English and French. French language skills are mandatory.
  • Must provide and maintain a satisfactory criminal record check.
  • Access to a vehicle and a valid driver’s license.
